YOU WILL NEED THE FOLLOWING DOCUMENTATION WHEN TURNING IN YOUR APPLICATION.

  1. Last 2 years W2 Forms and Income Tax Returns
  2. Last Four (4) employment pay stubs.
  3. If receiving child support we will need a print out from the Attorney Generals office
  4. If applicable: Award letters from Social Security or SSI
  5. The two (2) most recent bank statements (Checking and or savings).
  6. Last statements on all open accounts (EX: Visa, Mastercard, Sears, or loans)
  7. Valid drivers license and social security card
  8. If divorced we will need a copy of the final divorce decree.
